ところが、会談が終わって間も無いというのに、バイデン大統領が習主席を「独裁者」（dictator）と言い放ったということで、波紋を呼んでいます。

"off-script"というのは、台本（script）に無い、あるいは外れた、という意味かと思います。

政治家のスピーチや発言というものは、その内容が批判や揚げ足取りの材料にならないよう、よく考えてなされるもので、特にスピーチなどでは官僚や側近が準備した原稿を読み上げるものですが、"off-script"はそうした原稿が無い状況でなされるということを言わんとしているのだろうと思われます。
